1.
Your
First Visit
On
your first visit, please bring:
Your insurance card
Personal
identification
List
of current medications (prescription or over-the-counter) including:
Name
Dosage
Frequency
Name
and address of your current primary care physician
Referral
from your primary care physician, if required
Lab
reports, x-rays and any medical records, if available 2.
Office
Hours Our
hours are 9 a.m. to 5 p.m., Monday through Friday. Appointments can be made by calling (210) 616-0410 during
office hours. Patients are seen
by appointment only. We make
every effort to be prompt and available at your convenience.
If you are unable to keep your appointment, please cancel as far in
advance as possible. 3.
Emergencies During
office hours, call our office. For
emergencies occurring during the evening, weekends, or holidays, again call
our office. The phone is
answered by a twenty-four hour exchange which will contact us. We would appreciate your calling us before going to the
hospital emergency room. Many
times the problem can be handled in our office, being more convenient and
less expensive for you. One of
us will always be available to you.
4.
Telephone
Calls Please
make all phone calls that are not absolute emergencies during office hours.
For routine questions, we prefer not to interrupt other patients who
are seeing the doctor in the office. Our
staff has been trained to answer most questions.
If they are unable to answer your questions, it will be relayed to
your doctor. We return our calls as time permits or as the situation
demands. Should you call, we
ask that you give all necessary details concerning your problem to the
receptionist. In this manner,
your problem can be related intelligently and quickly to the doctor for
prompt response. It is not our policy to treat illnesses over the phone.
If you are ill, please plan to see us.
5.
Medication
Refills Please
direct refill requests to your pharmacy.
The pharmacy will contact your physician for authorization.
Only emergency prescriptions will be called in after normal hours and
weekends. 6.
Payment
for Services Patients
are expected to pay at the time service is rendered. A copy of your charges will be provided the day of your
visit. We accept payment in the
form of a personal check, MasterCard, and Visa. 7.
Medical
Insurance Your
medical insurance is an agreement between you and the insurance company.
It is our policy to provide to you, without charge, an itemized
statement that can be submitted to your carrier.
But, as a courtesy, our office will submit your claims for medical
benefits. We accept Medicare
assignment and participate with most insurance plans.

Referrals If
you are insured by a managed care plan, it is your responsibility to obtain
a referral from your primary care physicians.
